Disney's The Little Mermaid Film Fashions Paper Dolls, Golden Books, 1997, 12 3/4" tall.  Includes Ariel and Prince Eric paper dolls, punch-out tote, punch-out mobile on back cover, and 4 pages of punch-out outfits, none of which have been removed from the book.  There is a stain on the edge of two pages, see photo 3, 11 and 12 (photo 12 shows the back of the page that the Ariel and Prince Eric dolls are on).  There is bending and wear on the corners, see photo 10.  There is a price tag on the front of the book, see photo 1.
